So I thought I made one that was simpler for everyone (there's a lot of precise and great ones out there), some visuals and more easy to use.
And here is how it looks:
Pretty straightforward. Name in the white area, Numbers in the gray area.
List your monthly income (full time job, part time job, dividends, etc) in the "Income" box.
List your expenditures (phone bills, credit card bills, food, transport, etc) in the "Expenses" box.
List your assets (House, stocks, bonds, unit trusts, CPF) in the "Assets" box.
List your Liabilities (Home loan, School Loans, Total Credit Card debt, Personal loans, etc) in the "Liabilities" box.
Try to keep a monthly record to track how are you performing, or at least do a yearly one (but more data compiling).
And when you know these better, can proceed on to the more complicated ones =)

EDIT (08/04/2015): Updated an error in the cash reserve months (it shouldn't show a $ sign) and made the CPF asset more clear, just to include OA+SA, as they would be more useful to show the actual value you have which you will have for retirement (CPF is an annuity).
